All permits require an on-site person over the age of 18 that is responsible for the permit activity. Please provide their contact information and upload proof of age.
Please provide a brief description of the event, including its importance to the District or the region, and indicating any special needs to make it successful. If you’re requesting multiple dates, please list the additional dates, times, and participant counts below.

To demonstrate eligibility for permit rate and allocation priority, please attach any relevant additional information about your organization including: participant rosters with last names, birthdates, zip codes and scholarship status to verify quantity and DC residency, IRS determination letter, as appropriate. Please upload proof of age for the on-site person responsible for the permit activity.
